Your Apppointment

Prior to your first appointment there will be an initial discussion and conversation regarding your goals for therapy and to provide an opportunity to ensure that you are going to receive the most suitable treatment and duration of the session for your needs. This does not count as part of your Initial Consultation.

During your first appointment, additional time is given for a thorough Initial Consultation to ensure your treatment will be safe and effective. This Consultation also provides an opportunity to obtain important details and contact information for your Next of Kin, GP and any treating Medical Teams and to explore your medical history to highlight all contributing factors to be taken into account during Therapy. 

The Consultation will also focus on any additional emotional, physical and social considerations. Using this information, an individually tailored treatment plan will be made, discussed and agreed before starting the session.

You will be required to sign and date your Consultation Form prior to your Therapy starting.

This information will be checked at every future session with Chantelle Persaud Physiotherapy to ensure we have the most up to date information of your medical and emotional status.

To ensure Chantelle Persaud Physiotherapy continues to deliver high quality and safe treatments, there may be circumstances where we may not be able to provide treatment or we may require a GP/Nurse/Consultant Clinic or Correspondence Letter for Referral and Medical Clearance to ensure treatments are safe for you to receive.

(Fees may still apply in the circumstance you present for your session but treatment is unable to be delivered- Please contact us for further information)

We accept cash and bank transfer. In the event of a bank transfer, payment must be received prior to receiving the treatment and proof of payment must be provided.

Full payment will be expected in the event you arrive late for you session or cancel within 24 hours except where there are mitigating circumstances.
